Good Faith Estimate Notice
Temperance Psychotherapy LLC
You have the right to receive a “Good Faith Estimate” explaining the expected cost of your care.
What This Means
Under federal law, healthcare providers are required to give clients who are not using insurance an estimate of expected charges for services.
This applies to all private pay (out-of-network) therapy services.
What’s Included in a Good Faith Estimate
Your estimate will include:
The cost per session
The type of services you may receive
An estimate of how many sessions may be recommended
Because therapy is individualized, the total cost will depend on your specific needs and how long you choose to continue.
When You’ll Receive It
You’ll receive a personalized Good Faith Estimate:
After reaching out and scheduling
Before your first session
Or upon request at any time
Your Rights
You have the right to:
Receive a Good Faith Estimate before starting services
Ask questions about your estimate
Request an updated estimate if your needs or frequency change
If you receive a bill that is at least $400 more than your estimate, you may have the right to dispute it.
